Bug with disappearing inlay

I put the inlay which gives poison downward strike into my euradayle diresong and it just dissapeared from the game

While trying to put gems into slots on my new items. the First attempt to put a gem into each slot caused me to lose the gem altogether. The second gem was applied correctly. I tried this consecutively on 3 different items and lost 3 gems
